The Lancaster Model (TLM) gives your child a safe, confidential way to share their health needs — and connects them with the right professionals, sooner.

What is The Lancaster Model?

TLM is a health assessment tool used in schools. Your child completes an age-appropriate questionnaire online, which identifies any health needs or concerns and connects them with school health professionals who can help.

It was developed by Kath Lancaster to give every child the chance to express their thoughts, register worries, and access support early — in line with their rights under the UN Convention on the Rights of the Child.

What TLM does for your family

How does it work?

Each questionnaire is completed online through the TLM Portal. It uses research-based questions to build a picture of your child's health and wellbeing.

Once submitted, the school health team reviews the results and can follow up with advice, referrals, or support — just as they would with any health information.

You complete this when your child starts school. It covers their medical history, development, and lifestyle.

Your child completes this with support at school. It checks in on their health, safety, and how they're feeling.

Have you visited A&E following an accident or injury in the past year?
Have you had a dental check-up in the last year?
Does your child have a long-standing illness?

Completed in Year 9 and again after turning 16. Questions become age-appropriate for older teenagers, covering topics like emotional health and lifestyle choices.

How many people in your class do you think drink alcohol more than once a week?
